Students MUST complete a VA Form 4-572 Change of Address Form and turn it
into the CU-VA Office for processing BEFORE the CU-VA will change a
student's address. Changes submitted to VA-Muskogee MUST also be given to
CU-VA or it will cause confusion about your correct address.
The CU-VA Office WILL NOT change a student's address for VA purposes
without completion & receipt of a VA Form4-572.
KEEP THE CU-VA NOTIFIED OF ANY CHANGES IN YOUR NAME, ADDRESS OR
TELEPHONE NUMBERS.
VA Form 4-572 may be obtained from the CU-VA Office.
