Off Campus Proctor Application

This form ONLY needs to be completed by online students who reside 30 miles or more from Lawton/Duncan.

Pre-approved proctors are:

  • Proctor U (Students must have a web camera, microphone and reliable internet to use this service),
  • College or University Testing Centers,
  • Military Base Education Testing Centers, and
  • Sylvan Learning Centers.

Students are individually responsible for any testing fees incurred with ProctorU or any other testing center.

PLEASE COMPLETE A SEPARATE FORM FOR EACH OF THE COURSES YOU ARE ENROLLED.

STATEMENT OF AGREEMENT: THANK YOU FOR SERVING AS A PROCTOR FOR CAMERON UNIVERSITY By serving as a Proctor for Cameron University, the proctor hereby agrees to abide by the Assessment instructions below. The proctor will also recognize and respect the confidentiality of the faculty and the institution regarding all assessment materials and the confidentiality of the student.
  • Students must show one of the following types of governmentally issued, current picture identification: Drivers license, Military ID, Passport
  • The Proctor will verify the student’s CU identification number (which will be provided to the Proctor by the instructor or the Distance Learning Coordinator)
  • The Proctor will enter the Assessment Password (which has been provided to the Proctor by the instructor or Distance Learning Coordinator). The password will NEVER be shown to or given to a Student.
  • Students may NOT have personal items at Assessment station (i.e. Cell-Phone, PDA, Palm Pilot, Purse etc.) The proctor will require students to surrender any such items before the Assessment will be administered. be shown to or given to a Student.
  • Calculators, note paper, books, and notes will only be permitted with Instructor's advance written permission.
  • Under no circumstance will students be permitted to print, copy, or remove Assessment material from the Assessment premises.
  • Interpreters for hearing and vision impaired students must be approved in advance by the instructor.
  • Proctors will observe students taking online assessments.
