1. Application/ Financial Need Analysis Form with FAO Signature;
2. Certificate of Indian Blood showing at least 1/4 Indian blood;
3. Proof of Tribal Enrollment;
4. Student Essay;
5. Two letters of Recommendation;
6. Current Financial Aid Award Letter and
7. Most recent Transcript.
