Disability Services - Accomodation Requests

How to Request an Accommodation

  1. After completion of admission and enrollment, students should contact the Office of Student Development to initiate their request for services. A request for an accommodation based on a documented disability should be made prior to start of an academic semester or as soon as possible to ensure timely provision of necessary accommodations.
  2. Students should read the Information Sheet (available for download in Acrobat Reader format) and fill out the Request for Accommodations form (available for download in Acrobat Reader format). Once the Request for Accommodations form is complete, the student should contact the Office of Student Development to schedule an appointment with the Director of Student Development to discuss the need for accommodations.
  3. Students must provide proper documentation of their disability and their need for an accommodation before receiving services. The documentation must contain the following information:
    • Specific diagnosis of each condition/disability from a licensed or properly credentialed professional,
    • Information on how each condition/disability affects the individual with regard to physical, emotional or cognitive limitations and the severity and limitations within the classroom environment,
    • And may include specific recommendations as to the types of reasonable accommodations needed (e.g. extra time for testing, note taking assistance, special table/chair).
  4. The student and the Director of Student Development will meet to discuss reasonable accommodations. Please note that reasonable accommodations will be based on need, not desire. Services are intended to provide the student with equal access and opportunity in the academic setting.
  5. After the meeting, the Office of Student Development will provide the student's instructors information regarding the need for accommodations. Students receiving accommodations should identify themselves to their instructors to discuss a plan for accommodations. Students are not obligated to disclose specific disability information unless they so choose.
