- Employee coverage for life and medical.
- Faculty and professional employee coverage for long-term
disability.
- Optional coverage for employee and dependents available
through payroll deduction.
What is the amount of life insurance policy?
- Regular classified and professional employees- two
times annual salary rounded to next even one thousand dollar.
- Regular faculty- two times annualized salary rounded to
the next even one thousand dollars.
- Changes to policy amounts are made annually each
year based on salary in effect on Sept. 1.

When can changes be made to my health insurance
plan?
- Changes in plan or coverage are made during annual
open enrollment period.
- Certain qualified changes in a dependent's status allows
an employee to make changes during an option period.
What is Section 125 Flexible Benefit Plan?
- Provides pre-tax payroll deductions for certain qualified
insurance plans elected by employee.
- Medical savings account available.
- Dependent care savings account available.

Services, Inc. for Health Choice Plan.

- HMO claims are processed by the HMO designated claims
office.
